 When you sign your name at the bottom of the form you are agreeing to the following:

Photo Release: Gustaf Adolf Lutheran Church has my permission to use my child’s photograph publicly in materials. I understand the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.   

As legal guardian of the participant named above, I hereby give permission for this child to attend VBS at GALC and be involved in all activities. I also authorize the assigned Gustaf Adolf staff and volunteers to provide emergency medical care should any emergency occur while my child is attending VBS. Furthermore, in giving permission for my child to participate, I agree to pay all expenses resulting from such an emergency and in no way hold Gustaf Adolf Lutheran Church or its staff members/volunteers liable.
